In summary, we have taken the following steps to protect your privacy:

1. We follow all guidelines set forth by the Health Insurance Portability and Accountability Act of 1996 (also known as HIPAA).

2. We do not share your contact or health information with anyone except:

a. As required by law,

b. When needed by other healthcare providers participating in your care,

c. With your insurer when you request services to be covered by an insurer.

3. In the case of minors or for those who have legal guardians: protected health information may be shared with the parents/guardians of patients.  Information of this nature is not given out over the telephone or any other electronic media, except in cases of emergency or if permission is granted to do so in advance.

4. Hard-copy medical records are released only with the written permission of the patient or legal guardian.
